Requirements for Undergraduate Program

INCOMING FRESHMEN

To be admitted, all incoming freshmen should pass the admission test conducted by the Guidance Office and Testing Center, the interview of the Dean of the College that the applicant is planning to get admitted to the approval of the University Registrar.

» Form 138 or High School Report Card

» Certificate of Good Moral Character

» Two (2) copies of 2x2 colored picture

» Medical Certificate of accredited physician

» One (1 ) Long brown envelope

TRANSFEREES

The University admits transferees from other educational institution provided that they have complied with the requirements and procedure provided above for incoming freshmen and the following additional documentary requirements;

» Honorable dismissal

» Original Scholastic Records from the institution enrolled in

CROSS ENROLLEES

Cross-Enrollees from other educational institution for any academic subjects and/or any of the NSTP components are required to submit the following documentary requirements;

» Cross-Enrollment Permit from the home school duly signed by their Dean

» Letter addressed to the Registrar requesting that the cross-enrollee be admited stating therein the reasons for such request

» One (1) Long Brown Envelope


Requirements for Graduate and Doctoral Programs

DOCTORAL PROGRAM

» Original Transcript of Records reflecting completion of A Master of Arts Degree in an educational institution duly recognized by CHED, reflecting therein a valid S.O. number.

» Two (2) 2x2 colored pictures

» One (1) Long Brown Envelope

GRADUATE SCHOOL

All applicants to the UNEP Graduate School are required to submit the following documentary requirements;

» Original Transcipt of Records with a valid S.O. Number

» Two (2) copies of 2x2 colored picture

» Completed Application for Graduate School

» One (1) Long Brown Envelope

COLLEGE OF LAW

Documentary requirements for admission to the UNEP College of Law are as follows;

» Original Transcript of Records proving completion of a Bachelor's Degree in Arts and Sciences and reflecting therein a valid S.O. number

» Two (2) copies of 2x2 colored picture

» Certificates of Eligibility to take up la

»  One (1) Long Brown Envelope


Grade School Admission

KINDERGARTEN

Children seeking admission for Kindergarten should be at least (4) years old by the time he/she enrolls in the particular school year.

GRADE 1 to 6

As a general rule, all applicants are eligible for admission and be officially enrolled.

Old Pupils:

»  The University of Northeastern Philippines (UNEP) Report Card (UNEP FORM 138) for last school year attended

New Pupils:

»  Original Photocopy of the Birth Certificate

Transfer Pupils:

»  Report Card (Form 138) of the pupil from the school where he was previously enrolled

»  Birth Certificate

»  No transfer pupil will be accepted for admission to the sixth grade

HIGH SCHOOL

Freshmen/Transferees seeking admission in UNEP High School Entrance Examination:

»  Form 138 or Report Card

»  Certificate of good moral character issued to the school last attended

»  Photocopy of the Birth Certificate (NSO copy)

»  Copy of at least 2x2 pictures (colored)

»  1 short size brown envelope

Old Students:

»  Form 138 or Report Card

»  Must settle old accounts with the cashier,if any

»  If permitted to enroll in summer class in another school, the student must submit the card and Form 137-A with a passing mark in the subject taken to the Principal Office Secretary
